DSW – Waiver Job
Description:
This position is responsible for providing individualized services to developmentally disabled adults.
Principal Duties:
A. Implement individualized plans that may include services such as hygiene and grooming, housekeeping chores, cooking and marketing, personal shopping, and community activities.
B. Document services provided on the required form and entries in case notes.
C. Inform program staff of any incidents/accidents and submit a report.
D. Maintain consumer confidentiality.
E. Ensure consumer dignity, human rights, and personal safety.
F. Maintain professional conduct at all times.
G. Be available to work a flexible work schedule in the delivery of services to the consumer.
H. Have the ability to provide services to a diverse population/disabilities.
I. Other duties as required by the Director of Waiver Services or designated staff.
J. Participate and support the agency’s activities and work required to meet the agency’s goals and objectives.
K. Comply with federal, state, and company policies, procedures, and regulations.
Supervision:
This position is under the direct supervision and is responsible to the Director of Waiver Services.
Qualifications:
A. Current CPR and First Aid certification
B. Possess a valid Hawaii Driver’s License and have a clean driver’s abstract
C. Access to a reliable and insured vehicle
D. Clean driver’s abstract
E. 40 hours of volunteer or paid experience working with persons who are developmentally disabled
F. Patience in working with persons with disabilities
Education:
A. Prefer a high school diploma or equivalent
